The Monthly Treatment reports are due on the 10th of each month, along with your provider invoice.
We have two versions available for you to download. The Reports can be downloaded as Excel Worksheets, or
as .ZIP files.
FYI:
If you have Windows XP you should be able to download the WinZip version without installing any additional software. In addition, the Excel Worksheets were created using Microsoft Excel 2003.
